Better Listener LLC ("Producer"), producer of Chef On The House
By submitting this form, I ("Participant") agree to the following:
1. Appearance & Recording. I consent to being photographed, filmed, audio-recorded, and otherwise captured during the Chef On The House event I am attending. This includes footage taken inside and outside the premises during the production period.
2. Use of Materials. I grant Producer a perpetual, worldwide, royalty-free license to use, reproduce, distribute, edit, and publicly display any footage, photographs, audio recordings, or other materials in which I appear, for any purpose related to Chef On The House, including but not limited to: YouTube, Instagram, TikTok, and other social media platforms; streaming services; television broadcast; press and promotional materials; and the show's website.
3. No Compensation. I understand I will not receive financial compensation for my participation or for the use of my likeness or voice in connection with the show.
4. Food & Production Liability. I acknowledge that I am voluntarily participating in a food preparation event. I agree to inform the production team in advance of any known food allergies or dietary restrictions. I release Better Listener LLC and its personnel from liability related to foodborne illness, allergic reactions, or equipment use, except in cases of gross negligence.
5. Minors. If I am submitting on behalf of a minor, I confirm that I am the parent or legal guardian of the minor and have the authority to grant this consent.
6. Electronic Signature. I understand that checking the box below and typing my name constitutes my electronic signature and has the same legal effect as a handwritten signature under the U.S. Electronic Signatures in Global and National Commerce Act (ESIGN).
7. Acknowledgment. I have read and understood this release. I am signing voluntarily, without duress, and represent that I am 18 years of age or older (or have guardian consent as noted above).
